Welcome to the CSDNR Health Services Department.
The Health Services Department consists of a team of experienced nurses, support staff and a District-wide Medical Director. There are health offices in each of our school buildings that provide first aid and emergency care to students and staff. Our nurses are also an integral part of your child’s health care team when medications or health conditions need management during the school day. We work collaboratively with all disciplines in the school system, to ensure that your child is in good health and is able to participate safely in all available programs.
School Immunization Requirements for the 2023-2024 School Year
DTaP (grades K-12)
5 doses of diphtheria and tetanus toxoid-containing vaccine and acellular pertussis vaccine (DTaP)
4 doses if the 4th dose was received at 4 years of age or older
3 doses if 7 years or older and the series was started at 1 year or older
4 doses Pre - K
Polio (grades K -12)
Meningococcal (grades 7 - 12)
1 dose for grade 7, 8, 9, 10 and 11
1-2 doses for grade 12 (1 dose if given on or after 16 years of age or 2 doses of vaccine with the booster dose given on or after 16 years of age)
MMR (grades K-12)
Varicella (grades K -12)
Hepatitis B (grades Pre - K -12)
Tdap (grades 6 -12)
**Only doses of Tdap given at age 10 years or older will satisfy the Tdap requirement for students in grades 6,7,8. Doses of Tdap given at age 7 years or older will satisfy the requirement for students in grades 9 – 12.
Physical exams are required for all new entrants and students entering grades: PK, K, 1, 3, 5, 7, 9 and 11th grade.
